How Medicare Works

Most people become eligible at age 65. You may also qualify earlier if you have certain disabilities.

Generally, you must be a U.S. citizen or a legal resident who has lived in the U.S. for at least 5 years.

How much does it cost? Most people get Part A (Hospital Insurance) for $0 if they or a spouse worked for at least 10 years. Part B (Medical Insurance) has a standard monthly premium (set by the government annually). Depending on your income, you may pay more (IRMAA).

What is covered? Medicare is divided into Part A (Hospital stays, skilled nursing, hospice) and Part B (Doctor visits, outpatient care, medical supplies, and preventive services).

What are my options? Once Part A and Part B are active, most people add additional coverage to fill the gaps. Here’s how it breaks down:

  • Medicare Advantage (Part C) An all-in-one alternative to Original Medicare. Bundles Part A, Part B, and usually drug coverage into a single private plan — often with $0 premiums and extras like dental, vision, and hearing.
  • Medicare Supplement (Medigap) Extra coverage that pays the “gaps” Original Medicare leaves behind — co-pays, co-insurance, and deductibles. No networks. See any Medicare-accepting doctor in the country.
  • Medicare Part D Standalone prescription drug coverage. Critical if you’re on Medigap or staying with Original Medicare. Skipping it when first eligible can mean a permanent penalty later.
  • Ancillary Benefits Dental, vision, and hearing coverage can be added standalone or bundled depending on your plan type.
Compare Medicare plans

Medicare Advantage is an “all-in-one” alternative to Original Medicare. These plans are offered by private companies approved by Medicare and bundle Part A, Part B, and usually Part D (Drugs) into a single plan.

How much does it cost? Many plans have $0 monthly premiums, though you must continue to pay your Part B premium. You will typically have co-pays or co-insurance when you visit the doctor or hospital.

What does it cover? It covers everything Original Medicare covers, but often includes “extra” benefits like dental, vision, hearing, and fitness memberships. Most plans require you to use a specific network of doctors (HMO or PPO).

Medigap is extra insurance you buy from a private company to help pay the “gaps” in Original Medicare, such as your 20% co-insurance, copayments, and deductibles.

How much does it cost? You will pay a monthly premium to the private insurance company in addition to your Part B premium. However, your out-of-pocket costs when you go to the doctor are often near zero.

What does it cover? It covers the “gaps” in Part A and Part B. One of the biggest benefits of Medigap is that there are no networks—you can see any doctor in the country that accepts Medicare. Note: Medigap does not include prescription drug coverage.

Part D is optional standalone prescription drug coverage. It is provided by private insurance companies approved by Medicare.

How much does it cost? You pay a monthly premium that varies by the plan you choose. If you don’t sign up for Part D when you are first eligible (and don’t have other creditable coverage), you may face a permanent late-enrollment penalty later.

What does it cover? Each plan has a “formulary” (a list of covered drugs). Plans are required to cover a broad range of drugs, but they place them in different “tiers” which determine your cost. This is the most critical section for managing insulin and diabetes supplies.
